Okay i have my receipt from last weekend....do i just post them to the address on the mastercard website or do i have to fill in a form?? I did not see any when i was at the cinema last week, or is there any form i can download to use??

According to the t's and c's you need an original form. It says they won't accept photocopies.0

They are usually on the paydesk in a Mastercard branded cardboard holder.
I saw them at Leeds and Huddersfield Odeons, next time you are in I would pick up a batch.
