I did a balance transfer on my card at0% until next year. I have nearly paid off the balance. If I spend on the same card will I still get 0% until the offer runs out

It won't change the BT offer, but you will pay interest on your purchases if you don't clear the entire balance.0

Normally the cards are structured so that it will cost you a lot more from purchases on a promo BT card0

They aren't really structured differently. It's just a consequence of the "no interest on purchases if you pay in full" thing. By running a balance, you will pay interest on purchases. It doesn't matter if you are running a balance in the normal way or you are running a BT balance.
These days payments are first applied to balances that have the highest interest rate (provided they've been statemented). So the good news is that once the purchases have appeared on a statement, if the OP sends enough money to cover them rightaway*, the interest will stop running.
(NB a couple of cards do now preserve interest free purchases despite running a BT deal. I believe Nationwide is one of them - but do check your own deal before relying on this.)
(* if there is a DD in place, be sure to understand how this will interact with any manual payments.)0
