Is this because i filled in the date electronically or is it because i did not put my name and kept DEFENDANT at the top ?

Please phone the CCBC, they can tell you exactly why, otherwise we are just guessing. Best get advice from the horse's mouth on this.

Yeah the error was down to the reference one of the numbers was wrong - false alarm.
