I tweeted them yesterday about the form but haven't received any reply. Maybe more should do it?

I also tweeted them...no reply...however their get out clause in the T&C's

Well, given they've taken the form away - for whatever reason - I'd say that's pretty low. I've personally tweeted them twice and they've ignored me. You've tweeted and they've ignored. Pathetic really in this day and age.
