Life book? Death book?

Slightly off-the-wall question but I wonder if anyone is aware of the book I am trying to locate. I read about it recently in an article and then stupidly threw away the article before I searched online.
The book in question is a book you fill in for after you have died. It includes important information such as where your assets are, what sort of funeral arrangements you want, etc., but also has information such as your prized memories (and, I think, information like your favourite songs, etc.). The idea of the latter is that the book is for people you leave behind, so they have something to remember you by.
The book was created by a woman who realised that if she died, her friends and family would have no idea about these things. I am pretty sure the book costs £19.99 or £19.95.
Does anyone have any idea what I am talking about?

Some of that would be covered by http://www.ageuk.org.uk/home-and-care/home-safety-and-security/lifebook/0 -

Thank you, in fact I already ordered that and it looks very useful for the practical aspect. The book I read about has this (the practical aspects of where your assets are, standing orders, etc.) but also has a lot more emphasis on the personal side of things. I think the author was trying to address the idea that when you are gone, friends and relatives might be comforted by a book that contains elements of who you were. I'm not explaining it very well, but if I find it, it will be clear!(Nearly) dunroving0 -

It would be quite easy to do that - a pretty folder and some very good quality clear plastic sleeves and then add whatever you want to tell the people who survive you. That also allows you to change your mind!
We did something similar for my parents' 60th wedding anniversary - although this was made of contributions from family and friends - and it's been lovely to look back through since they died.0
