Final forms for divorce?

Hi. When it comes to submitting a final order to the courts, agreed by both parties, what form is needed to be completed to list assets etc (I know its NOT a form e).

For my divorce, which was an agreed clean break split, we each had to fill in Form A "Summary of Means", a lot less detailed than Form E.0 -

A consent order is not bullet proof if the OH contests it in the future after the divorce but having one does provide certain protection.
