BPA Parking Charge Notice - Date of Issue too late?

Please could someone advise me on how best to respond to a PCN?
I parked at Holy Trinity Church car park in Newquay, operated by Parking Eye, on 11 Apr entering at 1144. I elected to pay by phone which was emotional and took nearly 10 mins. I received no confirmation receipt but my credit card was charged £3.80. The PCN does not acknowledge that I paid any fee, it merely shows that I entered at 1144 and left at 1516.
The PCN was issued on 3 May and received on 5 May. Having read the newbies thread I believe that they have taken too long to issue the PCN as there was no ticket issued.
Should I use the standard template or should I specifically quote this lapse?
Any assistance would be most gratefully received.

just use the standard blue text template on their website , as keeper
yes they are out of time for pofa so may have omitted the paragraph that contains the pofa details
do not reveal who was driving and edit post #1 above accordingly
the driver parked
the driver paid by phone
the driver had no confirmation receipt
etc
ps:- attach evidence showing the charge was paid0
