Closing bank account - what information is required

Find out the balance, write a cheque for that amount made out to a UK charity, and post it to them.
Once the cheque clears, close the account with a nil balance.
Closing an account with a non-nil balance adds the complication of "what to do with the money?". Sort that out beforehand, and everyone's life (you and them) is easier.
