I Just got charged a STANDARD PURCHASE INTEREST £49.64 by Capital One VISA
seems excessive to me , I use it in Italy every few months inc cash advances my question is what is this and is it a one-off annual charge

It's the interest on your purchases. You're charged interest when you carry a balance and don't clear in full.
You're charged interest on cash from date of withdrawal.
