Halifax PLC - Data Protection

Hi,
I'm trying to reclaim PPI paid on an old mortgage I had with the Halifax from 1989-1995. I have provided lots of information but there is clearly some information they require that I haven't yet provided. They have confirmed they hold a record they believe relates to me but they need to verify 3 out of 5 points for Data Protection purposes which I understand. I've written back twice with additional information but still no joy. Does anyone know what the 5 points are for Data Protection purposes?
