is this the correct info to ask for if PIP assessment is a FAIL

thanks for all the help in the other thread. can someone please comment on this advice we have been given? we have been told that we should request ALL of the following information if the PIP assessment fails ends in failure?

advice given is the following:

request for copy of the original PIP claim (Or if that was rejected) including enclosed supporting evidence(if applicable)
• copy of the inter-department correspondence passing the case from DLA over to PIP
• copy of PIP2 questionnaire including all attachments and the Special Delivery bar code label
• copy of the ATOS/Capita PA1 file note determining that the case was to be handled via face to face assessment
• copy of the ATOS/Capita PA4 assessment report
• complete copy of the PIP 7001 award letter
• anything/all they hold regards your DLA/PIP claim
Computer records
Clerical records On top of that I would ask for the persons, be
it a nurse, gp, or other hcp their medical number and a copy of any record of their field of medical expertise, I

For PIP if you use the form ask for everything from the first call made to start the claim
  • PIP 01
  • PIP 02
  • PIP 04
  • PIP 7006
  • The GMC/PIN or other number of the Health professional who did the assessment
  • The expert qualifications of the HP, and their field of expertise
  • The medical qualifications of the Decision Maker
  • All the information the HP/Decision maker used to make their opinion/Decision
  • The medical expertise and GMC or other number of anyone the DM asked for help in the decision of the report
  • All the info used by the HP including websites they used to gather info prior to the assessment starting
  • All the info used by the DM plus websites used to check information in the report

    If you have been informed that you aren't entitled to PIP you need to wait for the written notification and then request a Mandatory Reconsideration IF you have grounds to believe the information provided in the notification is inaccurate. You would need to explain which parts are wrong and why. As has been said above, you do not need the information you have listed.

    If she needs to appeal, all relevant information for the tribunal will be supplied in the DWP evidence bundle which she will get as a matter of course.
    This stretches to around 80+ pages and includes her original PIP form, the medical assessment, any medical opinions sought, the DM's reasoning, her MR, the DWP's MRN, her SSSCS 1 form, the DWP's submission to the tribunal.

    If she really wants to worry about a possible appeal before getting the decision letter, she could spend her / your time becoming more familiar with PIP and the appeal process. Here are some resources:

    There should be a note at the top of the board warning new (and regular) posters about unsolicited messages of "advice". It's not the first time I've seen this posted, and I wonder how many other users are receiving these messages and acting on them without checking.

    I'm sure the sender of the messages is well intentioned, but he frequently posts information that is misleading and at times entirely incorrect.

    As I said earlier she is where she is and it may well be best to await the result of the assessment. After that she will have a chance to go for a Mandatory Reconsideration - and if that is unsuccessful - then she goes to appeal

    It is quite unusual for decisions to be overturned at the MR stage (it does happen) but 60% of decisions that go to appeal are changed

    Honestly - others may disagree -I'd be inclined to hold fire. She may well get the right outcome and you have 30 days to get the MR in after the decision letter if necessary

    But I am also posting for others who may have received similar advice. And I am repeating what I said on the other thread (as has Alice) to try to get the point across to him.
