County court claim form from Civil Enforcement Ltd

I have just received a County Claim form from Civil Enforcement Ltd. This is due to getting a parking notice from Parking Eye back in April 2016. The reason for the parking notice is overstaying a Burger King car park for long than a hour. I was about a hour and an half due to my girlfriend breastfeeding our son in the car before going in to Burger King. Also there was building work going on so the signs weren't very visible without looking in detail for them. I ignored all the letters and parking notices as that is what I was advised to do. I wish I had seen this forum and others like it at the time. I have been on the moneyclaim website and registered. I am just wondering which box I tick for my defence.
Do I tick the
I intend to defend all of this claim
Or
I intend to defend all this claim
And
I intend to contest jurisdiction
Please could someone advise me on this and also about what to include when writing my defence ?

This is covered by bargepole in the 'Small Claim?' section of the NEWBIES thread, you need to read his post to know how to fill the forms in and to be ready for the later, vital stage of Witness Statement and evidence (prior to the hearing).

Cowboy parking companies such as Civil Enforcement seem to have gained greater strength by careless rulings from a couple of out of touch judges that seem to have set precedents. I got a Parking Charge Notice (which is in essence an imitation or fraudulent Penalty Charge Notice) from Civil Enforcement after parking on a slip road on a suburban road in Cheshire. At 10.00 pm! Absolute madness. I have told Pizza Express (where we eating that night) to lean on their landlord to sack off Civil Enforcement - otherwise they will start to lose business. The fact that private companies such as this are able to access DVLA info to get names and addresses is outrageous. Which idiot in government allowed this to be legal? Probably happened after lobbying from the parking cowboys. When are people in this country actually going to stand up against these cowboy bullies?0

On the rest of the paper it asks for address, job title / employer and a few other section.
