Parking Eye PCN Best Online Appeal Template Help Pls

Im getting a bit confused with the best template to use for an Online Appeal to Parking Eye to either get the PCN cancelled or receive a POPLA number.

Ive seen about 3 different templates that can be used!! Can anyone point me to best one to use pls.

I actually have 2 separate PCNs for different dates but assume I can use the same template for each?

All help appreciated and thks in advance.

    Lovely thks guys.......does the bit about obtaining DVLA data still apply once received the 'PCN' as they have already obtained the data? Or am I getting cofused by something else/legal jargon?!!
