Which is, for the purposes of the form, fraudulent.

The OP is unemployed with an income of £0. That's the only factual way to represent their current situation.

    This is my worry.
However, let's say I get a 1 week contract that is based on a 40k salary.
In that week, I'd be in full time employment, with a salary of 40k a year, a week after that I'd be unemployed with a salary of 0. So not really sure how that would work either, as I'd know the contract was going to end. So you could say that is fraudulent too.

No you wouldn't, you'd be in employment for 1 week earning £770

In which case, how do I state an annual income!? Any contract could get extended or terminated early, therefore changing the goal posts.
The only way would be to average out my annual income over the last whatever many years but I'm not sure that would be defined as a "truthful" current annual salary.

I'm self employed / work for my own limited company. I have applied for credit cards recently, I just use the figure of my tax return for profit for the previous year on my SA302. That way I can prove the figure.

The problem I have is that I left my umbrella company in July as I went away for a few months but have just rejoined them, as I am now back looking for a contract.
I notice a lot of the application forms request how long you have been with the company... Technically, I've been a member for over 2 years (they still hold my data) but with a small break!
