Cleaning up disputed debt and ones from identity fraud

Ok so back in 2008 I got a phone contract and a few months they changed terms and kept telling me I couldn't cancel, after about 30 phone calls they agreed to either let me leave or drop my then current tariff to £15 a month and I wasn't held to the rest of the time period (which was another year) so could leave at anytime, about 2 months later I decided to leave so gave 30 days notice and paid and assumed the end.

A few times over the years I got contacted claiming I owe the remaining 12 months of contract at the non discounted rate, and when proof was asked for was just a copy of payments I made and them saying "well contract was 24 months and you paid 12, therefore you still owe 12 months"

I just ignored it knowing it would be statute barred as of 2015, except it still shows as a closed account on my credit account with money owing which I assume is because Lowell claim I made payments up to October 2013 which is common lies I hear about them.

I have another debt which is even more complicated, I was victim of identity fraud due to unsafe letter box during 2009-2014 and had a lot of credit applied for in my name, only checked one credit report and only one shows which is vodaphone (via Phones4u) the complicated part is at the time I had a unauthorised direct debit taken from account and bank refused to refund (bad informaton really) a few months back when I noticed I contacted Vodafone who claimed it was not fraudulent as 1 payment was made from me (the one I disputed with bank) and during that month phone was replaced due to it being faulty by sending a new one out.

Except the address was wrong i.e I was Flat D and it was say Flat A which was a neighbour who was a junkie and I knew was commiting identity fraud against me and other people.

I want to get these two fixed (as well as any other that arise due to identity fraud)

Whats best way to go about it?

    Send a "prove it" letter to Lowell. If they are saying you made payments up to October 2013 they need to prove it otherwise the debt is Satue barred.

    Second one is a lot more complicated! These things are dealt with much easier at the time but if you didn't know about it there obviously was not much you could do at the time. I'm really not quite sure how you can proceed with this one.
