PIP Application advice

I believe that I will shortly have to go through the dreaded DLA to PIP conversion procedure - others in my area have just received the paperwork to fill in
My problem is that I cannot write
I have an email address for DCS DCPU Alternative Format Team who claim that when the time comes - they will email me an editable application form - but would it be best for me to do this (thus proving that I can survive doing things like this without any assistance) or get the CAB to fill it in for me (similar) or should my wife fill it in for me ?
Also - the DCS DCPU Alternative Format Team - tell me that if I ask for an alternative format (ie Editable PDF) - Then that is the only format that DWP will accept from me - if I need advice from CAB - Then they won't be able to fill in an alternative form for me !!

My sister lives 15 miles away from me, and had a PIP invite letter in September.
I haven't had the invite letter yet.0 -
I can't write - I can't hold a pen, so I just dictated it on a normal PIP form and my son filled it out. Husband has dementia so couldn't really do it for me. My son then signed that I had dictated it but signed by him. Was accepted just fine. This wasn't a dla to PIP form, I hadnt' claimed DLA before.
My advice would be get a normal type form then others can do it for you but make sure dwp know who filled it out.Master Apothecary Faranell replied, “I assure you, overseer, the Royal Apothecary Society dearly wishes to make up for the tragic misguidance which ended so many lives. Is there a copy of the PIP form available on-line - so I can see EXACTLY what to expect ?
I know that there are many sites with lists of the questions - but I need to see an exact copy to get my mind around how I am going to answer them .....0 -
