You need to read the NEWBIES FAQ sticky, post #1 only at this stage. Send the template initial appeal (the one in blue text) around day 26 as per instructions on the ticket you've received. Don't alter or add to the template. Don't be in a rush to deal with this, follow the guidance.
In the meantime, you need to be in touch with PALS (Google it), preferably face to face if you're close enough to the hospital, they should be able to help - especially given the lies spouted by OCS; they trade-as Legion, they are Legion.

you squiggle a signature and send it by day 26 , first class post with a free certificate of posting from the PO counter staff (not recorded)
it is deemed "arrived" 2 days later (so by day 28) , so day 25 or day 26 is fine for posting as long as you do the above0 -
