Passport renewal query

I'm renewing my passport for the first time since the online service was introduced. I've paid the fee and downloaded the paperwork to be completed and sent in with new photos, but I have a question about the Declaration Form. It contains boxes for me to sign and date, as expected, but also includes boxes headed "Relationship of applicant to intended passport holder" and "Name of applicant applying on their behalf". Given that it was clear throughout the application that I was applying for a renewal of my own passport I can't see why these boxes are even present, but given that they are should they be left blank?

In case it's of any relevance, I have dual nationality (granted NZ citizenship, and I have an expired NZ passport) which may have put me into a different path in the online process from someone with just UK citizenship.

    Early on in the form?

    It sounds that you are looking at the section about countersignatories.I think you need to read thru the forms again as they are pretty straight forward.

    No and No. The PDF that I downloaded at the end of the application contains six pages, two guidance pages, the Declaration Form (one page), a blank page, and two pages on photo guidance. No countersignatory is required.

    The Declaration Form has what appears to be a form number on it, DCCL/01/05/02, googling that found this, which is exactly the same (apart from my details) as my download, so if you look at page 3 you'll see what the form looks like [I'm not sure why someone would put their passport application online for anyone to see, but it's fortuitous that they have].

    That's the bit you fill in if it's a child's application that needs parental consent. You don't need to fill it in. It's not the best laid out form.

    I have applied for a renewal of my and my partner's passport at the same time, as the website appears to allow you to make multiple applications on behalf of more than just yourself. I too cannot understand why they are asking for "relationship to applicant to intended passport holder" and "name of applicant applying on their behalf" without stating that this is only when applying for a dependent or similar. I completely understand your confusion Agrinnal as I too have been fathoming my way round this. We came to the conclusion that we probably need to leave this blank. This ought to be clarified in some kind of guidance notes but I can't find any. We are sending off leaving it blank and keeping our fingers crossed!

    Also, they only last week had a different online application process that meant you didn't need to send physical photos in but simply attach a photo file showing yourself from just above the waist upwards and stating that the passort office would crop the photos themselves. Now you can only print off and post headshot pictures 35 mm x 45 together with your declaration form(s) so been faffing around for ages doing those. Should have done it last week!

    The advisory notes could a bit clearer. My take on it is that it allows for helpers to assist e.g. the applicant could be visually impaired and or have different physical impairments.

    Update: Just received a response from HMPO and they have confirmed these boxes are "in case you are signing on behalf of a child or someone that for whatever reason cannot sign for themselves".

    I'll bet they didn't say why the boxes are there at all when you've already confirmed at a previous point that it's your own passport that you are renewing and therefore the boxes have no relevance whatsoever to your application. It's just sloppy coding on the part of whoever developed (or specified) their online application system.
