It also says the following on the thread:

NO NEED TO USE A REAL SIGNATURE - BUT DON'T POST UNLESS YOU HAVE TO - USE THE ONLINE APPEAL PAGE FOR FIRMS LIKE UKPC AND EURO CAR PARKS & PARKINGEYE, BECAUSE THE APPEAL CANNOT GET LOST AND YOU CAN KEEP A SCREEN PRINT. USE THE EMAIL APPEAL OPTION IF FIRMS OFFER IT.

As mentioned earlier I have pre-filled out the online form and ready to submit but unsure what reason to out for appealing from the drop down menu provided and to submit without evidence?

Nothing that admits fault from your part. So what choices allege the PCN was issued in error, or similar?
PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).

I submitted the appeal using the reason "other" option from the drop down menu.

That then gave me the option to paste the template letter adding my relevant details to the explanation box.

Hope this was the correct thing to do?
