I can't see any " Enter Passcode" boxes the form is there to fill in but when you press "Submit" the submit button just scrolls and nothing happens. Am I missing something?

Even if you are signed in and your number is correct at the top, you may need to select 'Not you?' next to your number and re-log in. Then it should ask for details then the code, if you can't remember your 4 digit code then select 'forgot passcode' and it will text you with a new code, then you can make your own memorable code up for the next time. Your entry should then go through.
