dla child global delay development

My child is 2.5 years old. Is under paediatricians care. She has delay in all 5 areas in her two year old asq score done by the hv. Her gentic test show she has chrmosome i disorder. A duplication on her chrmosome. She only says few word. V nouse sensitive hates water.can be aggressive and jumps all day and sleep very few hours.
Im applying for dla as the hv has said she needs more care than a normal child.
Portage slt and esrly education are in the pipelines.
Can any one help with example of whst to put on the dla form.
Form needs to be back 6th jan and most of the advice grouos are shut.
I need to explian how she needs more help. O just dont know how to put it into words

You could see if the DWP will give you an extension so you can get help from CAB/whoever0 -

I seem to recall that for very young children, it is not just the additional care that is important but also the nature and type of that care. So explaining fully what you do for your child (and its importance) is helpful.
If you can look at a copy of the Disability Rights Handbook (library?) this this explained more fully.
