ESA claim come to an end? Do I panic?

Just doing some filing and noticed the wording at the bottom of the most recent letter regarding my ESA claim. I'm in the support group (I have MS amongst other joys).
Normally they say something like 'the above amounts will apply from [such and such a date]'.
The most recent one says 'the amounts on this page apply from 30 January 2016 to 2 December 2016'.
As it's now 8 December 2016 I'm wondering whether I should worry that my claim has come to an end? I've not received any letters saying I have to claim again or forms to reapply and I can't find any documents with a cutoff date. Maybe there's a rate change? Not had anything about that either though.
Obviously I'll check with the job centre but was wondering if anybody had any advice: do I stay confused or up my state to panic?

Oh, phew! Many thanks for that; I had envisaged a flurry of form filling in my near future.0
