Lost driving licence

Asking this on behalf of a friend. Ordinarily, it is straightforward to apply for replacement, but this is the situation.
Licence in maiden name
Marriage cert in Italian
Has moved.
However, her passport is now in her married name. On that basis, is providing her passport number, NI number and addresses sufficient to apply for replacement as per Gov.uk?
