You do not state the direction of flow for the drain. Provided you have space, and adequate falls (levels would have to be taken to investigate this), plus a budget for the work then it is possible.
Being pragmatic, you need a detailed design with levels to know if you can achieve what you want together with a superimposed outline of the extension.0 -

I am at the end of the run no other sewer enters my manhole so flow is in the direction of the other house and about 600mm deep.The boundary line is at the edge of my house so can not move it to the left .Was hoping for a full width of my house extension and 4 meters depth ,the current sewer pipe is about 1.5 meters out from the house0 -

This is not feasible. The diverted drain would be re-routed on your neighbours back garden - would you get approval from them?
Your shallow drains suggest the levels will not work.
You are left with a build over if you can get approval for this.0 -
Do you need to move the actual sewer or just a manhole which is on your property?
Where are you also, as different water companies have different policies. I recently carried out an extension and had a manhole right in the middle of where I was going to extend to.
Southern Water certainly allow you to build over a sewer, but you will need to move the manhole. We removed the manhole and put a a new manhole with rodding access outside of the footprint of the extension.
Try googling your water company plus "build over agreement" which should tell you what you need to do regarding the sewer, then it is just a matter of getting plans and structural details together to submit to the water company. Cost approx. £300-400 for the agreement.0
