Barclays Bus. Account - Tax Compliance

Hi all,
I have a business bank account with Barclays and have been asked to submit a Tax Compliance form, however no-one i have spoken to knows anything about them? I have asked my Dads accountant, he doesn't know, he has then asked his bank and they don't know anything about it either? I think I will have to give Barclays a call, i'd thought i'd ask in here though as well to see if anyone else has had to fill one of these in? So here is my query:
I am filling in my Self certification Tax Compliance form, and the jargon relating the the Active/Passive NFE is a little confusing.
I run a small webdesign and support company, thus I have deduced that I am an Active NFE, however there are different types of Active NFE to choose from on the form, and this is where I am getting a little confused.
The options are:
Active NFE - Government entity (obviously this does not apply)
Active NFE - International - Organisation (this does not apply either)
Active NFE - Other (Ok, this seems like the right choice)
However, under Active NFE Other, it states 'Please refer to Guidance for further details' -= What Guidance? There is no accompanying documentation with the form? There is also a section under this one which states 'If you are a Start-up company formed in the past 24 months, please provide your date of incorporation'. I Incorporated in July, but have started trading, from what I have read this means that I am not classed as a start up company?
Could anyone help provide any clarification? Has anyone else had to fill this in?

What kind of services?
Any domain service for example which might have HQ in USA?0 -
Who has asked you for this form, the bank or somebody else?
In either case, your business account should have a dedicated relationship manager. Try asking them?: )0 -

My reading of the guidance is that your company (like mine) is indeed an Active NFE - Other.
For readers who have been lucky enough not to receive this message, it looks like Barclays is sending it out to new companies who have recently opened a bank account. I don't know whether it is only those who have dealings with the US, but that would make sense. However my older company, which also has US clients and a Barclays account, has not received such a letter.
