How open radiator lockshield with no spindle?

Hi, the plastic cap has a central screw, so when the cap comes off i see the screw fitting underneath not a spindle. On youtube, i only see examples where the spindle is directly under the cap and you use pliers to turn it but I am baffled what to turn. The screw thread seems to be in the same place but seems too fragile to "plier"
British Gas drained this radiator and it seems they closed the valve -now I have one cold radiator. i deliberately expired my homecare as my annual service in sept ended up with my boiler being broken for six days ie the engineer broke it - and he took four days with two supervisors to fix it. Though i am thinking they were supposed to check the rads as part of the cover, i am not paying them now to fix it.
