Certainly worth a try handing in a starter checklist but would still recommend you speaking to payroll about it. The new employer should be actively trying to find these details but from what you have said so far they seem not to be aware of this or cannot be bothered; so even giving them the form may not guarantee that they act on it. In my time in payroll I met many "payroll clerks" who had very little idea of what was actually involved in PAYE other than putting figures into the computer and getting them out.

With regard to the your actual tax position, on the details given so far it is impossible to say. You may have paid to much tax or possibly not. I asked earlier about a P45 but you have not answered that and to give an accurate idea of your tax position I would need details of this P45 along with details of your payslip with the 0T tax code.
Without these details all I can suggest is trying to get on the emergency tax code with your new employer and contact HMRC to hopefully speed up the process of them advising your new employer of tax code, previous pay and previous tax figures.
