BT Fibre broadband deal in July: Help claiming card and cheque / voucher

Hi all.
Hoping someone may be able to help - I took out a blockbuster BT fibre broadband deal that was advertised on MSE in July. The deal expired on 25 July and we ordered it on that day.
There were two seperate incentives as cash back. One was a credit card and the other was a cheque I think (or maybe a voucher). Only problem is, I didn't bookmark the page on MSE that the deal was on and I can't see it archived anywhere.
Does anyone know what the incentives were and how I would claim.

We'll try to answer your query as quickly as possible. When emailing, make sure you include your full name, BT user name, order date, connection date and the email address you signed up with.
