Policy covering £5000 just to cover the funeral cost, taken out around 8 years ago. If the insured gets a serious medical condition what should they do? Will this affect the policy? Obviously has to be disclosed yes?

no.

Indeed. There'd be little point in having a policy which an insurer could amend or cancel as soon as it looked like you were going to pop your clogs. Effectively it would only cover the most sudden deaths - the large majority of deaths (including early deaths) are not sudden.
So life insurance means that that the insurer agrees to pay out if you die within the next X years (or when you die, if it's a whole of life policy). That applies regardless of whether your health deteriorates gradually before you die, or you're struck down suddenly by a heart attack or car accident.
Provided you were honest about your health at the time you took out the policy there's no need to inform them of ongoing changes to your health, and they will have no effect on the policy.0 -
