Eventually (duh!) realised that I had a store card with the Arcadia group back in the dark ages. Having read some posts over the last few days, decided to call Santander today. The card was probably activated sometime between 1985 and 1990 and was active in 2001. All I had was one notification with an account number.
When I called, I was unsure as to whether they could access accounts so far back and was just hoping to find out to whom I should write. The lady I spoke to found the account, verified that there was PPI on there, asked me some questions, and set my complaint in motion. No forms, no request for paperwork, all done in about five minutes.
Don't know if I'll receive compensation or not, but they certainly made it less painfull than other companies I have approached.

I know for a fact you will not receive compensation!
If your complaint is successful you will receive back the premiums you paid plus 8% interest but no compensation
But the most important thing I hope you will have learnt is to not take out insurance you don't need or don't understand!0
