How many disabled spaces

My local paper is doing some articles about parking and I was wondering if someone knew if there was some sort of formula as to how many disabled spaces were required per site.
This case was a block of flats which has no disabled spaces at all.

This says (on page 3) ...
Employees and visitors to business premises
Up to 200 bays = Individual bays for each disabled employee plus 2 bays or 5% of total car park whichever is greater
Over 200 bays = 6 bays plus 2% of car park capacity
Shopping, recreation and leisure
Up to 200 bays = 3 bays or 6% of total capacity whichever is greater
Over 200 bays = 4 bays plus 4% of total capacity

Thanks for that Bod it's as clear as mud as with all these things.
The ladies position is not entirely clear to me, but it looks like she should go to the landlord and demand a disabled bay close to her flat and quote the Equalities Act.
It appears the Management Agents are allocating permit spaces, hers in on the 11th floor, and of course if she parks anywhere else she gets a ticket.
At the moment it is just being shuffled between the Landlord and the Agent, who seem to be dishing out tickets somehow.0
