Damaged Ocado Orders

Anybody else experiencing a decline in the quality of their Ocado orders recently?
I've had two weekly shops in a row with either a popped bottle of bleach which has contaminated several items in the same bag. Or a bottle of leaking rinse aid, which at first me and the delivery driver thought was popped but later transpired the cap wasn't screwed tight enough.
On both occasions items have had to be taken away by the driver due to the level of contamination. Driver insisted on taking nearly all the items in the respective bags but I insisted some of them could just be wiped down which he accepted.
On the latter occasion I had a email voucher for a free "grocery bundle" worth £20. Handily included in the email was a list of items in the bundle. When the order was delivered several items of the bundle were missing and neither were they included on the manifest.
Milk bottles and cereal packets seem prone to being bashed in compared to other supermarket deliveries too.
Fortunately used the email to get a refund on missing and damaged items. Customer service on the phone lines has been exceptional as always and are able to give goodwill vouchers for the inconvenience caused.
Ocado used to always be spot on for me and I rated them for customer service, quality and convenience factor.
Wonder why quality control slipped?

I have not experienced damaged goods. However, it used to be rare to have an unavailable / substituted item. Recently there has been the occasional one. Today I had 3 unavailables and 2 substitutions that were unsuitable which accounted for about half of the value of my order.
Also I was recently lured into signing up to Ocado Reserved by the offer of a £5 voucher. I asked for fortnightly deliveries, my account shows that I should have fortnightly deliveries but the system is creating orders weekly. So far there has been no explanation only the suggestion that I cancel the unwanted orders.0
