I had to help a friend fill in the form, she was struggling a bit, but now I want to know the next thing to do for her or suggest.
Once the form has been completed and returned, is it,
A: advisable to let her GP know
B: should she give the GP a copy of the completed form, ( I photocopied everything for her)
Thanks

No two ways about this one: Anything Free is not a Basic Right..it had to be earned...by someone, somewhere

She doesn't need to let her GP know or send him a copy of the form, but it helps if she includes any GP/Consultant/OT/Physio ect letters that she's been sent regarding her medical condition.0
