DLA Renewal form

Hi all
Does anyone know if the Child DLA renewal form is the same as the one when you first claim (DLA1A)?
Got our renewal pack through last week, and gave it straight to our sons teacher to fill in a page (Q20) quickly before school breaks up today.
Well they've only gone and lost it during this weeks tidy up. So just wondering if i can just download and print out DLA1A from https://www.gov.uk or if there's something different about a renewal form?
If so, i 'll have to try to get through on the phone a request a new one

As far as I am aware it is the same form as it is treated as a new claim.
Give them a call to check and if it is then you can download it here:
