Is there any grants to assist disabled moving into adapted home miles away?

I am a disabled single parent and have been offered a disabled adapted property 70 miles away which is closer to family, is there any grants loans or assistance available to help with the cost of moving??? i really dont know how im going to afford to move but i cant stay as im currently only able to use down stairs in my current home
