Hospital Income Plan (was Sun Life, then FirstAssist, now Cigna)

Hoping someone else bought this policy way back in the mists of time! It was probably 1994 when I took this up and I haven't needed to claim till now. The original policy booklet, which gave instructions for claiming and a list of hoops to jump through, has been lost.
I can't phone them (no voice!) but the email address FirstAssist (whose letterhead is on my last renewal) gave on their useless website bounced and I was told to send to [address]@cignainsurance.co.uk - they promise a 3-5 working day response time. But my admission is before that and they may want info from the hospital during admission.
Does anyone have a scan of the relevant bit of the policy document? I'd be very grateful!
