Help required with fraud template

Hi all
Hope this is an acceptable forum to post this request, please direct me elsewhere if needed.
I have fraudulent items on my credit file. I have discussed this with the collections agency, and with the companies the accounts have been opened with.
I know who the person is, it's a family member that has done this before. I have been advised by the companies if I can have this person sign a declaration admitting they have acted fraudlently without my consent, that the credit file notes and liabilities can be transferred to them and removed from my file.
I plan on taking a harsh stance with my family because they have done this before and now put my attempt of buying at house at risk.
Is there anywhere I can get a basic template for this? And is there anything else I should be aware of at this stage?

Hi Mikecc,
We don't have a template letter for the family member to complete but we do have a sample letter that you may be able to use (possibly slightly adapt) when writing to the debt collectors. You could adapt this sample letter when writing to the creditors to dispute the debt. Normally the creditors would also ask for a reference number from Action Fraud (the UK's Fraud reporting centre), whether this is necessary if the family member admits fraud I couldn't say (it is quite a unique situation for the person to admit the fraud).
The debt collector should then launch a fraud investigation. If it is concluded that you were the victim of fraud you should not be held liable for the debt and the details should be removed from your credit file. If the creditors believes it is your debt after their investigation then you can take the matter to the Financial Ombudsman.
Ultimately, if the family member refuses to admit the fraud and you choose not to report it because of the additional consequences this could mean for them, then I am afraid you are effectively accepting liability for the debt and would have to repay it and your credit file won't be changed. Good luck,
Laura
