Housing Benefit Review Form

I have just received one of these forms and whilst I am going to call them to find out why I have been sent it, I am a little stuck on one of the questions it asks. Hoping someone can help!
It asks for details of "Money that goes out. Please send in up to date proof". Then below says "Please tell me about things like child car costs, maintenance - not day to day living expenses". I don't have any child care costs as my daughter is at school and I don't use any wrap around care, my daughter lives with me so I receive rather than pay child maintenance. Other than general utility bills, rent, CT, insurance premiums etc, I don't pay anything else.
I had called them on 12/4/16 to confirm a small increase in salary as at that date. I was told that I would need to wait until I had received May's payslip as they need two months slips to make any changes. I wrote on 13/5/16 enclosing my original April and May payslips and a letter I had received from my employer confirming my salary increase as from 12/4/16. All originals as the woman in the helpline said they would not accept copies. I called them again on 3/6/16 to chase up the update but was told I needed to wait at least a full month before chasing.

One more question, I have a bank account for my daughter, a First Saver account titled "my name In Trust For daughters name". The account is titled this way automatically with it having being opened when she only 2/3 years old, she is now 6.
Do I have to declare this on the form?0
