Debt Management Company paid creditors late!?

Hi guys,
Hoping someone can help as i've no idea!
My other half is trying to get a mortgage sorted for a property, she's had debt issues in the past but used a debt management company (smooth, who then went into liquidation and the plan moved to moneyplus) to sort out payment plans etc.
She paid the company on time and in full every month until all of the debts were cleared (which she has bank statements to prove).
We've checked her credit file (with all 3 agencies) and it appears that the debt mangement company has failed to pay her creditors on time so they continued to show 1/2/3+defaults on her accounts far after the plans we put in place.
Can we get this corrected? She's done everything she can to sort out her previous problems but the debt company seems to have screwed her over?
