My ex-employer (a recruitment agency) has knowingly provided false info to HMRC

Isnt this illegal? My ex-employer has knowingly provided false information about me to HMRC. They have faked the following information on my P45 form:
(1) The date that they filled the form
The date that they've put is fake - I was still an employee on that date: I hadnt yet even sent them my email resignation letter at that time. So they definitely did not fill in the P45 form on that date. They are knowingly and deliberately providing false information. Fortunately for me, I have written proof of being employed by them at that time (i.e. I have chit-chat type emails that were being sent back and forth between us, discussing issues. I also have proof in my bank statements because I was paid on a weekly basis).
I find that their behaviour is highly unprofessional because on that part of the P45 form, it states: "I certify that details entered in items 1 to 11 on this form are correct"
(2) "Leaving date"
Again, the date that they've entered is deliberately fake, because I was still an employee at that time and have written proof.
(3) "Last entries on P11 Deductions Working Sheet:
"Total pay to date"
They have put " £0.00"
"Total tax to date"
- They have put " £0.00"
Is this a common thing that employers do?! How have other people tackled this issue?
I have the feeling that they are doing this because they want to skirt around the issue of paying me my annual leave holiday entitleme t that I had accrued. This money was not paid to me in my final payslip.
I specifically ensured to address the issue of "annual leave entitlement" by email, so that I have evidence that may be used later on, and Im glad that I did. I was informed that I will be paid annual leave entitlement after I have received my P45 form.
What would be some sensible next steps to take?
I have taken photos of my P45 form (and have also taken photos of the envelope because it shows the date stamp of when the letter was received). I plan to email the photos to them and say that the details in the form are wrong. I plan to do this ASAP because i only received my form in the post last night.
Does anybody else have any suggestions?
Should I inform HMRC?

Employers get p45s wrong all the time. So at this stage I wouldn't say they've done anything illegal as you don't know why it's wrong.
I'd go back to them first and see what they say.
Wouldn't speak to HMRC until after you know what's happened.0 -
