SSP query

I have been off work since janurary and have dropped down into half pay. I am due to have major surgery on 9th june and SSP runs out on 31st July and my half pay runs out on 31st August.
My work are trying to get me to come back and do a shift next week to get me back in the system so i can use annual leave for 4 weeks and then go back off sick for surgery therefore extending my half pay for another month to recover.
My question is when I go back off will I be entitled to SSP again? I know its 28 weeks but how big a gap do you need between claims.

Periods of sickness of less than 8 weeks are treated as one episode.
However when you are no longer entitled to SSP you will be eligible for ESA, you should get a form from your employer to claim this.Lost my soulmate so life is empty.
I can bear pain myself, he said softly, but I couldna bear yours. That would take more strength than I have -
