S2 advice - treatment abroad

hello
just wondering, if somebody can help me with this....
I am about to take my daughter for treatment abroad (Germany), as she can't get proper treatment here. On NHS website I have found S2 form - Funding for treatment in Europe application form. Has somebody ever had an experience with this? What is the best thing to do - to apply before or after the treatment, as there are 2 options. And how likely and quick you can get this money?
