– Pre-paid AVIS voucher must be issued
– Reservation and voucher must state rate, name and rate code

THE PRIZE
1. This Prize Draw is open to residents of the UK aged 18 or over, except for employees of Immediate Media, Lonely Planet and Prestige Holidays and their families, agencies or anyone else associated with this competition.
2. There will be one overall prize for one winner plus guest. The prize consists of return flights for two from London Gatwick, car hire and five nights BB accommodation at the Artemisia Resort in Sicily
3. The maximum total prize value is approximately £2000 based on the recommended retail value at the time of printing. The Promoter accepts no responsibility for any variation in the value of the prize.
4. The flights and accommodation must be taken in one trip, and both require 14 days advance booking and are subject to availability. Travel must be booked by end of 30 September 2016. Travel not permitted between 10 July and 25 August 2016
5. Prize travel is subject to availability. General Conditions of Carriage. The winner and their companion must travel together on prize flights and will not accrue any frequent flyer points on the prize travel. An upgrade cannot be purchased on airfares with cash or with frequent flyer points.
6. Immediate Media, Lonely Planet and Prestige Holidays make no representation as to the safety, conditions or other issues that may exist at the destination.
7. Compliance with any health or other government requirements is the responsibility of the prize winner and companion.
8. The winner and their chosen companion must comply with all relevant customs, security and immigration requirements, including appropriate passport and visa requirements. Immediate Media, Lonely Planet and Prestige Holidays will not be responsible for any passenger’s failure to comply with these requirements.
9. The prize winner and the companion are responsible for holding a passport with at least six months validity. And must organise relevant visas where required.
10. The prize does NOT include any insurance, travel to and from the UK departure airport, or any travel or other expenses during the trip. Any out of pocket expenses (such as phone, room service, drinks purchases, gifts and food are not included in the prize.
11. The package is non-transferable and no alternative packages will be offered.
12. There is no cash alternative.
13. The competition prize is subject to availability.
14. Immediate Media, Lonely Planet and Prestige Holidays shall not be responsible for any indirect, special or consequential damages or losses suffered by the entrant or a third party.
15. The competition is provided by Prestige Holidays.
16. The winner will be selected at random from all entries. The Promoter’s decision will be final and binding. No correspondence will be entered into.
17. Winners will be notified by the prize provider by email within 28 days of the competition closing date.
18. If we can’t contact the winner within a month of the closing date, we will offer the prize to a runner-up, or re-offer the prize in another promotion.
19. You must provide your full name, address and daytime telephone number. We will use your personal details to administer this promotion, and will not publish them or provide them to anyone without your permission.
21. You agree that if you win, we can publish your surname and county in the magazine.

On Chrome the boxes come up with a white square (grey outline) when clicked. I guess an issue with the image they use for the box fill. As long as you click each box (other than the last one) only once, you should be fine. Should reject if you don't agree to T&Cs.
