ESA appeal tribunal

Anyone here know how the main ESA appeal process and the tribunal works? Both in general and in relation to the specifics below:
1) What do you have to submit with your initial appeal papers to lodge the appeal? Is it a very basic application and you don't have to go through the whole "why are you appealing", evidence etc just yet?
2) Do you only have to think about the mentioned evidence and why you're appealing when it actually gets to tribunal, assuming you're having an in person appeal?
3) Is there a minimum notice period for the appeal tribunal? I read 2 weeks somewhere but that could just have been one person's experience rather than a general rule. If I didn't have to get my case together until the tribunal itself and had two weeks to solidly work at it that would be fantastic. Not sure I can cope with it now though.
4) Any ideas how long it's taking to get a tribunal date these days?

I suspect getting a tribunal date varies from area to area but, in my experience, it is taking a number of months at the moment0 -

To add.
You will get a choice of oral or paper hearing. I believe you will have 14 days to respond to the enquiry form which asks you some information including whether or not you want an oral hearing. As I understand it you are more likely to be successful at an oral hearing
If you choose the oral hearing you should get 14 days notice of the date0
