Default issues and removing from credit report

Just applied for a mortgage and been declined by some and only accepted by Halifax with 15% deposit only because I have a default on my credit report which I don't think is my fault.
Basically had a loan, went on maternity, called them to see if I could pay reduced for one year, later on in agreement as I have been paying a certain amount they reckon they sent me a default notice to say I need to pay more anyway phoned up and agreed 80 a month (they sent this Jan 2014) so I paid £80 Jan Feb March (25th) then on the 28.03.16 they closed my loan and sent it to metropolitan collection services and called me 7th of April to say they done this and give me the creditors details to arrange a new payment plan. They seem to think this default notice in jan covers what they did? I cant remember recieveing this but I must of as I called them to sort it so therefore as I called an arrange a new payment plan this original default notice would of been invalid as the default amount on the Jan would not of been the same as 28.03.16 as I paid 3 payments of £80. I am awaiting response from them.
