PIP 2nd tier tribunal

Hi, I am in process of writing an appeal against the tribunal's decision on PIP based on error in law. I'm struggling to find a template or format of the letter and how to address it to whom in order for my appeal to be considered at 2nd tier.

You would probably be best going to citizens advice and getting them to do it.
I take it the court have given you permission to take it to the upper tier, Also are you aware of the laws etc for PIP.0
