Default incorrectly on credit file

Hello, newbie question so apologies if it's been covered previously.
Wonder if someone can advise the best form of action?
In January 16 I noticed a linked address I didn't recognise listed on my Equifax credit file for a contract with O2 taken out in August 2015. I contacted Equifax who said the data was entered correctly and that I needed to take this up with O2. I contacted the O2 fraud department and eventually having been messed around the FOS. This resulted in an undertaking by O2 to delete the credit file entry and issue a cheque for £25 as a good will gesture. In March I was surprised to see 2 defaults had been placed on my file resulting in a 140 point drop in my credit score. This again listed the unknown address and the same O2 contract debt, now owned by a debt collection agency. I have been in contact with all parties. Equifax refuse to help. Debt collection agency said it may be a miss-trace but now insist the data is correct. O2 are not replying. What am I supposed to do and how can this even be possible?
To confirm. The debt is not mine. I have never been a customer of O2. I have no association with the linked address having been resident elsewhere for over 10 years which a simple electoral registry search would confirm. To say I am annoyed with the treatment I have received from all parties is an understatement!
