LPA forms- making corrections?

We are completing the LPA forms for mum and we have just realised that her witness for life sustaining treatment hasn't written his name and address in block capitals.

He also witnessed mum's donor signature on both LPA but has done those in blocks.
What is the best way to correct the part that isn't in capital letters?

    Oh gosh, are they very picky?


    I did plan to phone tomorrow, Monday, but I think I was looking for a 'don't worry' response as I'm quite agitated by it and blame myself for not keeping an eye on things.

    Given that everything has, or should have, had everything signed and witnessed they then write to all the signatories and ask if they are satisfied with everything. If you need a copy the tell you to go and get it certified by a notary public. When you do this they expect you to write a declaration on every page and sign it. Need I go on?

    I have just completed it! The process of getting all the signatures in the right order with a short time scale was a real PITA. Take it steadily and good luck. Treble check everything as you go. Preparing it online and then printing it out is the easiest way.

    If you get something 'wrong' when you submit the forms you might forfeit your fee and have to pay again when you resubmit.
    Very much worth double checking before you submit.
    I know deep sighs.....
