Advice needed - bank taking advantage?

Hello all, long time lurker - first time poster as I'd like some advice on the situation that happened over the weekend.
At the company my girlfriend works for, her and another girl share the same name - lets call them Mary 1 and Mary 2.
On saturday, my girlfriend (Mary 1) checked her bank (remaining unnamed) account and saw that she had not been paid at all, all her scheduled direct debits for the 1st of the month had been deducted on the mini statement and she was massively into an unauthorised overdraft. In a complete panic she went into town to go into the her branch and find out what was going on.
After a brief wait she was taken into a customer services advisors office to go through things. The net result was the bank advisor informed her she could not explain why she had not been paid and that they would/could (you decide) not honour any of her direct debit payments and they would all bounce, they would not temporarily grant an extended overdraft to cover this while it was sorted out and the only way to immediately remedy this would be to extend the loan she already has with them.
As one of the direct debits is towards our mortgage she was in an absolute state and agreed to increase her loan. I also now see that the t&c's state that as its not a new loan but an amendment to an existing one that the normal cooling off period does not apply - theres no backing out.
What actually happened is that Mary 2 changed her bank, informed the employer of the change and the employer went and changed Mary 1's BACS payment details by mistake. The payment was rectified this afternoon.
So now my girlfriend has been paid (3 days late) but is facing god knows what charges and has and extra loan that she doesn't want and now doesn't need. As an aside the loan money hasn't even been paid into her account yet either.
I'm utterly disgusted at the banks actions and I'd like any help that can be offered in the best way to having this loan removed - I have the bank in questions complaints procdure, but thats about the sum of where I'm at. We're also furious with her employer and would like to know how best to approach them, but without raising formal complaints etc.

Her employer made the mistake and should reimburse any costs incurred. I don't think the bank acted incorrectly they offered a solution to a problem that was not of their making and your girlfriend accepted it. Ask them to cancel the additional loan or if they will not do that see if you can pay it back without penalty.

I would say the solution to this is simple - employer is in breach of contract and is therefore liable for the costs girlfriend incurred at the bank.0

It sounds like the Bank acted pretty reasonably in the circumstances - Needs to talk to employer, talk to Bank and see how to untangle this.0
