Hello,
My daughter is 16 and would like to change her Middlename to her Forename?Would like to change her Passport,Bank,GP etc).What is the best way to go about this?

She isn't changing her name, just altering the way she uses and writes it.
No need for a deed poll or anything else. Just tell the GP/Bank etc. If she wants a new passport she will have to apply for a replacement.
My mother has used a middle name as her "first" name for most of her life, so the name she uses doesn't match her birth certificate - never had a problem with any passport application or other documentation.0 -

She has two names, she can use whichever she likes - she isn't actually making a change.0

We wanted to 'officially' use her Middlename as her forename....The Passport Office won't just change her names around!Same at the bank...we tried and the latter said the names in the passport and in that order will be used.0

The only thing to be careful with is to make sure that the name on her passport matches any flights booked. My mother has always been known by her middle name, but her passport is in the same order as her birth certificate. When a friend booked flights for both of them she used my mother's middle name and my mother had to pay for it to be reissued so it matched her passport.. . .I did not speak out

But what does it matter? Both names are on there, she just uses the middle one not the first one in daily life... I don't see the issue?

If she wants to make a formal change she will need to do a change of name ded / deed poll.
Alternatively, she can simply use the middle name for day-to-day.
My dad has been known by his middle name all his life. His passport shows the full name, bank just uses initials
You can normally specify how you want your name to appear on your bank cards, she could probably have (say) 'J. Elizabeth Smith' or 'J.E.Smith' or Jane E. Smith'
As your daughter is 16, if she does want to make a formal change then the deed will need to be signed by her and also by one of her parents, or she can wait until she is 18 and do it just on her own signature.
If she does make a formal change she will then need to provide her change of name deed as well as her birth certificate for any situations where a birth certificate is needed.All posts are my personal opinion, not formal advice Always get proper, professional advice (particularly about anything legal!)0 -

If it's just her it may well be a teen fad which will probably pass when she enters the adult world. Meanwhile she can use whichever name she wants to be addressed by. Just leave the official documents alone as it's hardly worth the hassle. How many of her friends and acquaintances will see her birth certificate or passport?The questions that get the best answers are the questions that give most detail....0
